Our Philosophy

Your trust is our greatest asset. We make every reasonable effort to ensure that information held by Mozart.MD-powered websites and apps is confidential and secure. We disclose our information-security practices to you as clearly and fully as possible. These practices receive continuous oversight, and many of our business decisions, including web design, technology selections, and third-party business relationships, begin and end with consideration for your privacy.

While our philosophy will not change, the details of the policy will change over time as we add additional services and form relationships with new business partners. Security

Mozart.MD has implemented security policies and rules to protect your account credentials, personally identifiable information and personal health information under our control from unauthorized access, improper use or disclosure, and from unauthorized modification. We use all reasonable means to provide secure transmission of your information from your computer to our servers, and to maintain the privacy and security of your data physically stored in digital form.

Encryption

When you transmit personal information to us, we encrypt it using Secure Socket Layer (SSL), the current industry-standard encryption technology. Encryption provides a secure means to protect your information as it passes over the internet to our servers.

Firewalls

Our servers and other technical infrastructure are protected from network intrusion using firewalls and other means.

Internal Access

Our employees and contractors have occasional, legitimate needs to access our data servers for purposes of system maintenance and troubleshooting. We ensure that such access is granted only to those who have such needs and for legitimate purposes. All such individuals have signed confidentiality agreements with Mozart.MD and are continually made aware of their obligations regarding user information. Access is controlled via preassigned user accounts that require multiple levels of authentication and only the minimum amount of access necessary is permitted. Additionally, all staff are periodically trained regarding security protection of their personal workstations.

Use of Cookies

A cookie is a small piece of text sent to your browser by a website you visit. It helps the website to remember information about your visit. Mozart.MD uses cookies to maintain your session state (the fact that you are logged in) and to report user interaction patterns on our site so we can continually improve the user experience. These cookies are used to store non-personally identifiable information, with the exception of the optional "Remember me" feature, in which you may explicitly choose to have your email address stored in a cookie as a convenience for quicker login to future sessions. All other personally identifiable information, your password, and your personal health information will never be stored in a cookie.

Personal Information

Mozart.MD Online Intake will collect some personal information from you about yourself and others in your family, for the purpose of providing your healthcare provider(s) with comprehensive information regarding your personal and family health history prior to your visit.

Account Credentials

We will never share or disclose your account credentials (password) with others or reveal it in any email communicaton.

Personally Identifiable Information

When you submit your Online Intake form, your full name, email address, birthdate (if you have entered it) will be viewable (but cannot be edited) by your healthcare provider(s) or staff members.

Personal Health Information

The personal health information you provide in your Online Intake form will be viewable (but cannot be edited) by your physician(s) or staff members. Their acceptable use of this information is described in their Notice of Privacy Practices, which you have the opportunity to read and acknowledge before submitting your information to their office.

Advertising

Mozart.MD will not sell or share your personal information with any third party for the purpose of advertising.

Terms of Use

Mozart.MD may provide you with access to the Online Intake system and a variety of resources (collectively "Services"). The Services, including any updates, enhancements, new features, and/or the addition of any new Web properties, are subject to the Terms of Use.

Legal Disclaimer

Unfortunately, no data protection method or combination of methods can be guaranteed one hundred percent secure. We take all reasonable steps to protect your personal information as described throughout this policy, but we cannot ensure or warrant our ability to do so. As a result, you use Mozart.MD content, products, and services at your own risk. Mozart.MD will not be liable for disclosures of your personal information due to errors in transmission or unauthorized acts of third parties.
